Dear Parents:
If your child has asthma, allergies or a seizure condition and may need to use an inhaler, Benadryl, epinephrine delivery device (EpiPen or AUVI-Q)  or Diastat at school, please contact our school nurse Susan Schriver, RN to discuss with her your child’s needs. Some parents have already spoken with our nurse so if you have you can disregard this request.
All medication given at school except Tylenol and Ibuprofen require written parental permission and a medical provider’s written order on the NSD District form, “Authorization to Administer Medication During School Hours”. The nurse will help you fill out this form and send it to the medical provider who prescribes the medication. The prescription information on the container itself does not meet that district requirement.
Please remember all medication except inhalers and epinephrine delivery devicesmust be brought to school by an adult. Please do not send your child to school with any other medication including cough drops or other non-prescription medications.
